GLOVIS America offers all regular full-time employees and eligible dependent(s) a comprehensive medical plan through CIGNA. We offer two medical plan options with different levels of copays, deductibles and out-of-pocket maximums. Employees have the choice to choose from the following: Open Access Plus Base $10 Co-pay or Open Access Plus Choice $15 Co-pay. In addition, GLOVIS America offers Health Care Flexible Spending Accounts.
GLOVIS America offers all regular full-time employees and eligible dependent(s) a comprehensive dental plan through GUARDIAN. The PPO UCR Plan offers services both in and out-of-network services; dentists will be reimbursed 90% of the UCR charges based on their geographical area and full coverage within the network. Under the PPO FFS Plan, larger out-of-pocket expenses occur when using an out-of-network dentist and less out of pocket cost for Preventative, Basic and Major services for in-network services. Both plans require $50/ $150 deductible and $2000 calendar year maximum.
GLOVIS America offers all regular full-time employees and eligible dependent(s) a comprehensive vision plan through VSP. The VSP vision plan features rich benefits with low cost, a 12 month frequency allowance on exams, lenses and contacts and a 24 month frequency on frames. Employees are given an allowance of $130 and co-pays are $10.
Life & AD&D Insurance
Company-paid Life Insurance coverage equal to two times annual base salary, to a maximum of $300,000 though GUARDIAN.
